Clifton Community Health Services (CCHS) is seeking a dedicated Clinical Nurse to join our dynamic General Practice team. About the Role

Professionally accountable to the Executive Director, the Clinical Nurse provides expert nursing care across a broad scope of practice. This includes chronic disease management, preventative screening, care planning, patient education, and team-based care delivery.

You'll also:

  • Coordinate and deliver health assessments, GP Management Plans (GPMPs) and Team Care Arrangements (TCAs)
  • Act as a clinical mentor and lead by example within the nursing team
  • Work closely with patients, families, and the broader care team to ensure integrated, culturally safe care
  • Support quality improvement and accreditation activities

About You

You are an experienced and compassionate Registered Nurse who thrives in a team environment and is passionate about improving the health of individuals and communities.

Essential:


• Current AHPRA registration as a Registered Nurse


• Demonstrated clinical competence in general practice, chronic disease, and preventative care


• Strong knowledge of MBS billing, care planning, and practice software (Best Practice desirable)


• Exceptional communication, documentation, and teamwork skills


• Proven ability to work both independently and collaboratively in a fast-paced environment

Desirable:


• Experience supervising or mentoring staff or students


• Current Influenza vaccination


• Familiarity with rural or regional health settings

Mandatory Requirements


• Bachelor of Nursing (or equivalent)


• Current National Police Check


• Up-to-date COVID-19 vaccination status


• Evidence of Hepatitis B immunity
