Social Worker – Older Persons Mental Health Service – Burnie TAS
- Full time contract up until 31st March

- Join a collaborative multidisciplinary team
ACM Healthcare is working on behalf of Government Health Service in Tasmania providing Older persons community mental health care in the North West. Join a multidisciplinary team delivering person centred, recovery orientated and trauma informed care, in accordance with National Standards for Mental Health Services.
RESPONSIBILITIES COULD INCLUDE:
- Information, support, and counselling for individuals and families.
- Assessment of mental health problems.
- Treatment advice and support in community and inpatient settings.
- Home visits as appropriate or required.
- Liaison and referrals with other health care professionals and service providers as needed.
- Community education and health promotion.
MANDATORY QUALIFICATIONS AND EXPERIENCE:
- Appropriate Social Work Degree or equivalent qualification and registration with relevant professional organisation
- Working with Vulnerable People Check must be obtained prior to commencement
- Ability to work until 31st March 2025
