Disability/Mental Health Support Workers Wanted in Melb South East

Attainable Care is a leading Registered NDIS/TAC/WorkCover Provider dedicated to creating a world of possibilities for individuals with disabilities. We are dedicated to providing a range of tailored services, including Supported Independent Living (SIL) and Specialist Disability Accommodation (SDA) homes.
We are currently seeking passionate and committed Support Workers to join our growing team, with multiple locations across South East Melbourne:
- Box Hill
- Berwick
- Noble Park
- Ringwood
- Bayswater
- Cranbourne
- Frankston
About the position:
Team Management
- Oversee and manage a team of support workers within service outlets or community settings.
- Collaborate with the rostering team to coordinate and optimize support worker schedules, ensuring participants receive consistent and appropriate care.
- Coordinate support worker rosters to ensure participants receive consistent and appropriate care.
- Handle participant and support worker cancellations, maintaining timely communication to fill shifts as required.
- Sourcing new support workers as needed and liaise with HR to expand the team when necessary.
- Managing availability of team members.
Staff Development
- Induct new team members, providing a comprehensive introduction to their roles.
- Provide tailored briefings, inductions, and shadow shifts for new team members to support onboarding and integration.
- Facilitate ongoing training, coaching, and mentoring for staff, with a focus on SIL/SDA-specific skills, and provide regular feedback.
Participant Care & Planning
- Work closely with participants and stakeholders to develop and implement person-centred care plans.
- Manage participant documentation, including medication charts, emergency management plan, and ensure all records are up-to-date.
- Schedule and coordinate participant appointments, including medical visits and allied health appointments, to ensure timely and efficient care.
Task Assignment & Operational Oversight
- Assign and oversee various tasks to team members, including cleaning, transportation, and client support activities.
- Conduct regular audits of SIL homes, performing quality checks with clients to maintain high standards.
- Oversee team members' performance.
Incident Response & Risk Management
- Respond to and report incidents as they arise, providing debriefing support and engaging internal resources as needed.
- Identify risks and implement control measures to prevent future incidents, promoting a safe environment.
- Conduct regular quality and risk assessments to uphold service standards.
- Report any disciplinary concerns of team member to the manager.
- Communicate housing-related issues to the housing team.
Communication & Staff Coordination
- Facilitate consistent communication among support workers to promote teamwork and effective collaboration.
- Manage communication channels (such as the Basecamp group chat and email) to ensure smooth and effective team operations.
- Attend care team and regular care manager meetings to ensure alignment on participant care.
- Serve as the primary point of contact for participants, the housing team, rostering, allied health professionals, support workers, and the management team.
- Maintain ongoing communication with the management team, involving them in key updates as needed.
Workplace Health & Safety
- Report Workplace Health and Safety (WHS) issues promptly, ensuring a safe and compliant work environment for all staff and participants.
