The Opportunity

The role of the Care Team Leader is to lead the team of Community Care staff who provide direct care to our clients. This is Permanent Full-Time position working Monday – Friday, located in the South East Sydney Home Care Team.

About the role

The role ensures that staff have a good understanding of individual client needs and preferences and can provide services tailored to the needs of individual clients, delivered within budget and service expectations.

Responsibilities include;

  • Leading, supporting, and mentoring their team, and works in partnership and consultation with the Care Manager, Clinical Care Manager, Client Liaison Manager, and Regional Area Manager, where required

  • Manage the recruitment of new Care Workers and ensure all working validations, education, development plans and resources are available 

What do you need to apply?

  • Certificate IV in Aged Care (Desirable), HR or similar degree & a minimum of 2 years relevant industry experience.

  • Knowledge of the Home Care Packages Program, CHSP services, Aged Care Quality Standards and Commonwealth Home Support Programme.

  • Performance coaching experience and management of a team while operating in a multi-site environment

  • A current driver's licence and safe driving record and willingness to travel between HammondCare's clients, sites, and services

  • Strong organisational, administrative and computer skills

  • Empathy and patience.
