Full Job Description

We're looking for a compassionate and experienced Care Coordinator to join our team. This is a casual position with an immediate start for the right person.

At Eagle Quality Care, we believe in supporting people to live independently and confidently in their communities. Based on the North Side of Brisbane, we're a growing NDIS provider dedicated to delivering high-quality, person-centred care with heart.

About the Role

As our Care Coordinator, you'll play an important role in connecting and supporting participants throughout their NDIS journey. Your responsibilities will include:

  • Coordinating and overseeing supports for NDIS participants.
  • Managing the onboarding of new participants, ensuring a smooth and positive start to their supports.
  • Liaising with participants, families, and support coordinators to ensure individual needs are met.
  • Preparing and maintaining staff rosters.
  • Supporting team members to deliver consistent, person-centred care.
  • Maintaining accurate documentation and ensuring NDIS compliance.

This role is ideal for someone who is organised, empathetic, and passionate about making a difference every day.

What You'll Bring

  • Previous experience in Care Coordination, Support Coordination, or similar within disability or community services.
  • Sound understanding of the NDIS and participant onboarding process.
  • Excellent communication and relationship-building skills.
  • The ability to multitask and work independently.

Qualifications & Compliance:

  • Certificate IV or higher in Disability, Community Services, Aged Care, or related field.
  • NDIS Worker Screening Check.
  • Working with Children (Blue Card).
  • Current First Aid & CPR certificates.
  • Valid Driver's Licence and reliable vehicle.
  • COVID-19 and flu vaccinations (as required by NDIS).
