ABOUT THE ROLE

This role is part of our Rural Health Team, which is a multidisciplinary team delivering Allied Health and Nursing services. Our target population is typically older adults (65+), people with disabilities or people with chronic conditions impacting on their daily function. Our services are delivered in peoples' homes, communities or are clinic based. We use a mixture of individual and group service delivery models and have a focus on wellness and reablement.

This position will deliver District Nursing services within the Home and Community Care Program for Younger People (HACC-PYP) and Commonwealth Home Support programs and within the Fee-For-Service stream.

In this role, you will provide a flexible and timely District Nursing service focussed on support for older frail or younger disabled people (or their carers) to be more independent at home and in the community. This support aims to enhance quality of life and/or prevent admission to an acute facility or inappropriate admission to long term residential care.

ABOUT YOU

You're an experienced, self-motivated nurse who thrives in the autonomy of community-based work and enjoys building strong, respectful relationships with clients, carers, and health professionals. You balance clinical excellence with compassion and can adapt quickly to the varied needs of a rural health caseload.

To be successful in the role you will have:

  • Tertiary qualifications as a Registered Nurse with current registration with Australian Health Practitioner Regulation Agency (AHPRA).
  • Demonstrated experience in community nursing and in wound care management, as well as an understanding of chronic disease management strategies.
  • Current driver's licence and willingness to travel across the region.
