Senior Pharmacist

Use your pharmacy expertise to support older patients in staying well at home. As a Senior Pharmacist in Toowoomba, you'll work closely with clinical teams to improve medication safety and continuity of care across aged care and hospital-in-the-home services. This role suits someone who values collaboration, consistency and practical impact in a rural health setting.

What your day will look like….

You'll review medications, support discharge planning, and liaise with health professionals to ensure safe, effective treatment for older patients. You'll contribute to telehealth services, visit patients at home when needed, and help shape pathways that reduce hospital readmissions. Whether coordinating with Geriatric Emergency Department Intervention teams or guiding medication changes, your work will reflect the values of compassion, integrity and innovation that guide Darling Downs Health.
