Care Manager – Westminster Village Residential Aged Care - Semaphore Park

Westminster Village is seeking a skilled and dedicated Care Manager to provide senior clinical and operational leadership at our Residential Aged Care home in Semaphore Park.
Reporting to the Residential Operations Manager, this key leadership role ensures consistent, high-quality care and strong clinical governance. You will guide and support the site team to meet Aged Care Quality Standards and deliver best-practice, person-centred care.
In this role, you will:
- Provide Clinical and Operational Leadership: Step into leadership roles at any of our five residential aged care homes to support during periods of leave, transition, or change.
- Drive Quality and Compliance: Ensure adherence to Aged Care Quality Standards, AN-ACC funding requirements, and accreditation processes.
- Mentor and Coach Teams: Build staff capability through supervision, reflective practice, and on-site mentoring.
- Respond to Clinical Complexity: Support the assessment and care of residents with complex needs, ensuring person-centred, holistic care planning.
- Ensure Care Continuity: Maintain clinical oversight and continuity of services, especially during transitional or challenging periods.
- Lead Improvement Initiatives: Conduct audits, identify risks, and support implementation of quality improvement plans.
- Participate in On-Call Roster: Provide after-hours clinical support across the aged care portfolio on a rotating basis.
To be successful in this role, you will have:
- Current, unrestricted registration as a Division 1 Registered Nurse with AHPRA
- Minimum 5 years' post-registration experience in aged care, including residential operations
- Proven leadership capability and advanced clinical problem-solving skills
- In-depth knowledge of Aged Care Quality Standards, AN-ACC, care planning, and documentation
- Excellent communication and interpersonal skills with the ability to lead multi-disciplinary teams
- Proficiency with clinical systems and electronic documentation
- Current SA Driver's Licence and NDIS Worker Screening Check (essential)
- Postgraduate qualifications in Aged Care, Leadership, or Health Management (desirable)
