We are seeking a passionate Registered Nurse to join our team at Korongee in the role of Clinical Support Nurse at .8 FTE.  This rewarding opportunity will enable you to demonstrate clinical reasoning, leadership, and staff education skills, to support and model contemporary clinical practice. This is a Level 2 role, which is well supported by on floor Registered Nurses, a Clinical Care Manager, and an onsite Facility Manager.

This role will enable you to focus on a clinical specialty area and fine tune your clinical education and mentor skills in a team environment. Be part of a management structure of likeminded professionals, to help us to strengthen our industry-leading approach to aged care service delivery.

Key Responsibilities

  • The role of the Clinical Support Nurse (CSN) at Korongee Dementia Village is to provide daily oversight and care coordination for Residents living across a small-house model.
  • The CSN will provide clinical oversight and care of Residents living across a defined area in the Village in a collaborative team approach, reporting directly to the Clinical Care Manager. Overall support and leadership will be provided by the Korongee Facility Manager.
  • The CSN works closely with other members of the clinical and care team to identify specialised care needs in our residents and works with the team to develop and deliver evidence based, undertakes care planning, supports admissions into the defined area and provides ongoing assessment and evaluation of health status working with the RN's and Clinical Care Manager.
  • Meet the required care needs for residents in a timely manner through collaborative communication with their representatives and the resident.
  • Work with the care and clinical team to identify specialised care needs and plan appropriate care to meet these needs in collaboration with the resident and/or their representative.

Knowledge, Skills and Experience:

To be successful in this role you will be required to demonstrate:

  • Experience in identifying, documenting, monitoring, and meeting residents changing needs using a holistic approach within an aged care setting, including palliative and dementia care.
  • Experience in assessing needs, developing, and evaluating individualised personal plans to meet those needs.
  • A high level of written communication skills and ability to document clinical care.
  • A high level of interpersonal communication skills and demonstrated ability to communicate effectively and respectfully with families, carers, GPs, allied health workers and other stakeholders.
  • Commitment to continuous learning and quality improvements.
  • Experience working effectively in a multi-disciplinary team.
