About the Role:
The Advice Coordination and Expertise (ACE) Inpatient Team supports patient care via the delivery of a service model that is committed to the principles of providing integrated care to patients.
Inpatient Care Coordinators are allocated to Medical Units within Western Health and are responsible for facilitating access to community services to ensure a safe and sustainable transition from hospital to home.
Western Health is committed to streamlining and responding quickly to patient needs, enabling them to easily navigate the health system. As part of this model of care, the ACE Care Coordinators are crucial in identifying clients and working with other services/programs to ensure that clients get the right service mix to safely support care in the community.
Key Responsibilities:
- Early identification of patient needs and potential barriers to discharge
- Ensure completion of timely referrals to both inpatient (iPM) support and post discharge (Central Access Unit) supports
- Expert knowledge of available community supports both internal and external to Western Health
- Innovative transition planning – In hospital, Rapid Discharge Support Service, Weekend Ward Service, Routine Post Discharge Follow Up Phone Calls
About You:
- Relevant acute and community related experience
- Extensive experience with assessment, discharge planning, case conference and care planning for people with complex needs
- Highly developed and demonstrated clinical assessment skills
- Sound knowledge of My Aged Care and NDIS

Western Health also provides the primary healthcare service at the Dame Phyllis Frost Centre, a women's prison located in Ravenhall.
