About The Opportunity
Are you a compassionate and experienced nurse passionate about End of Life Care? We're seeking a dedicated End of Life Care Nurse Practitioner to join our multidisciplinary team within the South Western Sydney Local Health District (SWSLHD) community. In this important role, you will lead and coordinate clinical care for patients in their final 12 months of life, ensuring they and their families receive respectful, compassionate, and person-centred support throughout their journey.
As an End-of-Life Care Transitional/Nurse Practitioner (NP), you will deliver expert, patient-centred care to patients nearing the end of life and their families, adhering to ethical, legal, and risk management standards. Your responsibilities will include assessing, diagnosing, and managing complex healthcare needs, alongside contributing to clinical research, education, and leadership. You will play a vital role in Advance Care Planning, ensuring care aligns with patients' wishes to minimize unnecessary hospital stays and promote quality of life, particularly for those vulnerable to frailty. Working within a multidisciplinary team, you will identify and coordinate end-of-life care for patients with chronic illnesses, frailty, and disabilities. This role is guided by NSW Ministry of Health policies, prioritizing safe, evidence-based, and compassionate care.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

- Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency, with a Nurse Practitioner endorsement in a relevant specialty field. Alternatively, a Transitional Nurse Practitioner (TNP) currently undertaking a relevant Master's degree that will lead to endorsement as a Nurse Practitioner, or willingness and eligibility to undertake a Master's degree that will lead to endorsement as a Nurse Practitioner. Applicants must possess the equivalent of three (3) years full-time experience in an advanced practice nursing role within the area of intended practice, obtained within the past six (6) years.
- Demonstrated ability to conduct comprehensive health assessments of target patient populations, collecting relevant clinical data to support evidence-based clinical, diagnostic, and prescribing decisions within a nursing context.
- Demonstrated ability to effectively communicate and engage with patients/consumers, carers, and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
- Demonstrated ability to perform in a clinical leadership role evidenced by participation in practice development activities including mentoring, education, active participation in communities of practice, policy development, research, and quality improvement.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
