About your opportunity:

In this position you will

Contribute to an innovative evidence-based model of health care for aged care clients with complex health care needs.

Effectively communicate at all levels across the organisation to promote effective team functioning.

Deliver quality, patient focused care co-ordination utilising specialist contemporary knowledge and recent clinical experience for the management of the frail and elderly in a community environment with the primary focus of establishing sustainable supports for the prevention of functional decline and avoidance of hospital admission.
