  • Focus on managing care teams, residential care staff and work closely with DFFH
  • A Full time, fixed-term contract until June 30th 2026 working across the southern division

Your role:

The Case Manager will oversee the care team of specialist professionals in delivering case management for high risk high needs young people.

The position objectives are:

  • Case Manager is to provide leadership and coordination of the care team for each young person placed in the Eastern region Residential Care program.
  • Provide support to young people in the program and lead the multi-disciplinary team to develop and implement trauma informed plans for each young person.

To Be Successful in this Position You Will Need:

  • Tertiary qualifications, preferably in social work / psychology, youth work or equivalent which includes diploma level qualifications.
  • Commitment to providing quality care and have experience in leading multi-disciplinary teams.

  • You get a salary starting at around $100,776 plus super
  • Salary packaging is available up to $15,900 plus an entertainment card.
  • SCHCADS award level 5.
  • Full time 38 hrs weekly, Fixed-Term until 30th June 2026.

  • Negotiable through the increment's dependent upon experience.
