We are looking for a Registered Nurse with years experience in mental health to join our team. You will be responsible for a caseload of up to 25 consumers, with opportunities to get involved in a Dialectical Behavior Therapy program & provide expert advice around physical health issues. Hornsby Ku-ring-gai Mental Health Services - Care Coordination Team, based at Hornsby Hospital

What you'll be doing

The Clinical Nurse Specialist Grade 2 (CNS2) provides a conduit to facilitate information sharing, planned care and care coordination for patients within the hospital and the continuum through liaising and networking with key stakeholders both internal and external.

The CNS2 is an expert in their area of speciality who provides leadership, advice and recommendations on all aspects of related care. The CNS2 formulates and implements policy, guides practice, and engages in education and research activities. The CNS2 acts as a role model providing complex care to those patients requiring advanced clinical skills regarding their area of specialty.

Candidates will need to meet the following criteria:

  • Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them
  • Division 1 Registered Nurse with post registration qualification relevant to Mental Health and at least 3 years experience working in Community Mental Health or clinical area of their post graduate qualification or such other qualifications or relevant equivalent experience.
  • Evidence of and commitment to patient/customer focused care, clinical governance and quality improvement
  • Demonstrated knowledge in initiating and implementing changes to improve clinical service delivery and outcomes, models of care, evidence based practice and practice development.
  • Demonstrated excellence in written and verbal communication skills and computer literacy in Microsoft Applications.
  • Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion.
  • Current NSW Drivers licence.
