My Place is currently seeking applications for a full-time Co-ordinator in the metropolitan area. The Co-ordinator plays a vital role in supporting young people in  the transition from Out of Home Care (OOHC) into services, such as the National Disability Insurance Scheme, once their formal care order ends.

About My Place

My Place is an established not-for-profit provider of individualised and flexible supports to people with disability and their families, including children in the care of the Department of Child Protection and Family Service. My Place supports around 400 people with disability to live in their own homes, or remain in their family home, and to become valued and contributing members of their community. My Place does not support any group home, day centre of other congregate care services.

About the Role

As a Co-ordinator, you will work alongside young people with disability, their families and allies to co-ordinate their OOHC and NDIS supports in an individualised manner. Key responsibilities include:

  • Meet with individuals and their support networks to identify goals and support needs and develop tailored transition plans.
  • Supporting young people with disability transitioning from Out of Home Care (OOHC) to NDIS services.
  • Liaise and advocate with NDIA and government departments, including navigating the appeals and review process.
  • Explore and research service approaches that align with the young person's needs and goals that meet funding expectations.
  • Maintain registers for foster care reviews, screenings and training.

About YouWe are looking for individuals who have:

Selection Criteria: Essential

  • Demonstrated contemporary or progressive attitude and values towards people with disability and children/young people involved in the child protection system.
  • Experience in disability, community or social services, or a related field
  • Experience in contemporary and innovative service design for individuals, and the implementation and ongoing management of individual support arrangements.
  • Ability to recognise and respond to the strengths and needs of young people leaving care, with a focus on maintaining supportive relationships and ensuring continuity of care.
  • Knowledge of and/or experience in planning, claiming and review processes within a NDIA environment, including managing individualised budgets.
  • Demonstrated well developed interpersonal, communication and written skills, including in the context of supporting consumers and team and staff supervision.
  • Competent in the use of standard computer packages and information technologies.
  • Ability to work autonomously from a home office, in the community or office.
  • Has a current Driver's Licence and WA Police Clearance. Co-ordinators will also be subject the National NDIS worker screening, Departmental and WWC Checks.

Selection Criteria: Desirable (highly regarded)

  • Previous experience in supporting individuals in a trauma informed way.
  • Knowledge of and/or experience in processes within an Out of Home Care (OOHC) environment (this is highly regarded)
