Clinical Aged Care Assessor (New Position)
Full Time – 38 hours per week | Fixed Term – 12-month contract

About the Role
This role supports the Single Assessment System by delivering Home Support and Comprehensive Assessments for older Aboriginal and Torres Strait Islander peoples across the Hume Aged Care Planning Region. Working under the Manager of Programs, you will provide culturally safe, person-centred assessments that value family, kinship, and community.
You will assess the care needs of older people under the Aged Care Assessment Program (ACAP), supporting access to services that promote independence, dignity, and community connection. Using a wellness-focused and strengths-based approach, you will partner with clients and families to understand their needs, goals, and preferences.
As part of the Positive Ageing & Disability Services team, you will complete timely and holistic assessments that consider physical, medical, psychological, cultural, social, and environmental factors. You will make recommendations in line with the Aged Care Act, ensuring cultural safety, respect, and self-determination remain central to care.
About You - Qualifications, Knowledge and Skills:
Qualifications and Registrations
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse, Physiotherapist, or Occupational Therapist , OR
- An approved tertiary qualification in Social Work with eligibility for membership with the Australian Association of Social Workers (AASW) .
Knowledge and skills:
- Strong understanding of Aboriginal and Torres Strait Islander communities, including the social, cultural, and historical factors that influence health and wellbeing, and the ability to apply this knowledge to provide culturally safe and responsive services.
- Knowledge of the legislative framework relevant to Aged Care Assessment.
- Ability to work co-operatively and effectively within a multidisciplinary team.
- Sound knowledge and competency in the principles and practices of comprehensive geriatric assessment.
- Excellent interpersonal, written, and verbal communication skills.
- Strong time management, organisational, problem-solving, and prioritisation skills.
- Demonstrated ability to work with computer systems to a high standard .
