Forward Ability Support is a not-for-profit organisation with 60 years history of providing key services to people with spinal cord injuries or similar related health conditions. We are driven to make a brighter and more positive future for our community by supporting people with spinal cord injuries to live their best life. Ferguson Lodge is our specialised accommodation site located in Lidcombe, providing support for those with spinal cord injury since 1979. We offer community based accommodation with a high level of personal and clinical care for those who have higher level needs.

The Opportunity

We are seeking an individual with NDIS experience to become the primary point of contact for all potential clients, their families and/or representatives. You will coordinate the intake process for services related to Ferguson Lodge, Community Care and Clinical Support.

The role is a Permanent, Full-Time working 8am-4pm Monday to Friday.

Your responsibilities will include:

  • Be the primary point of contact for potential clients', their families, local area coordinators and/or support coordinators to facilitate access to community resources
  • Accurately create quotes, record information for client case files, internal records and track funding
  • Determine clients' eligibility for Forward's services and refer potential clients to other more relevant services if required
  • Liaise with various stakeholders to develop quotes that meet the needs of potential clients and within Forward's requirement, ensure successful facilitation of NDIS plans and support client outcomes
  • Work within constraints of funding levels in line with each client
  • Conduct initial support assessments as needed and perform risk assessments with clients or their representatives
  • Ensure a positive and streamlined intake service

About You

You will be offered the right support to excel in this role, but we would love to see that you have:

  • Demonstrated knowledge of the NDIS framework and funding, iCare and DSOA
  • High level of professional communication skills, both written and verbal, to liaise with clients, their families and representatives and internal stakeholders
  • Experience in supporting people with disabilities in a community setting
  • Ability to work autonomously, set own priorities and work to deadlines
  • NDIS Worker Screening Check, or willingness to obtain one
