We're seeking a dedicated Wellbeing Manager to join our team and help transform lives across the Toowoomba Region and surrounding suburbs.

If you're passionate about creating positive change and want a role that offers both purpose and stability, this could be the perfect opportunity for you. This is a full-time, Monday to Friday position offering flexible working arrangements and access to a company vehicle. This role involves regular travel, with driving times of approximately minutes, so a love of travel is must.

A Day in the Life

  • Coordinate tailored Support at Home Package plans and manage care delivery, budgets, and clinical support to ensure holistic, person-centred care at home.
  • Advocate for clients in clinical settings and support transitions under the Support at Home program.


What you bring

  • Strong case management skills (ideally in community/aged care), Support at Home assessment experience, and a goal-oriented, holistic approach
  • Excellent communication and stakeholder engagement
  • Hold a current driver's licence and willing to travel up to 60 minutes within the region and surrounding areas


Position Details

  • Successful applicants must complete a National Police Check and provide proof of work rights in Australia
  • Full COVID-19 vaccination highly desirable
