About The Role:
Our in-home Nurses deliver specialised, high quality nursing care to older Australians in client homes in our local communities.
As a Community Nurse, you will provide person-centred care to clients living independently in the community. You will be responsible for:
- Conducting comprehensive clinical assessments and developing individualised care plans.
- Delivering 1:1 in-home nursing care, including wound care, catheter care and insertion, and safe medication administration.
- Supporting Home Care Package clients to maintain independence and enhance their wellbeing.
- Working collaboratively with clients, families, and multidisciplinary teams to deliver evidence-based, high-quality care.
We are looking for someone who is not only clinically capable but also compassionate, empathetic and committed to making a difference. You will have:
- Current AHPRA registration as a Registered Nurse.
- Experience in aged care or community nursing.
- Strong assessment, planning and clinical skills.
- Excellent communication and interpersonal abilities.
- A current driver's licence and reliable vehicle.
- Meaningful work supporting older people to live safely at home.
- Ongoing professional development and training.
- Supportive and collaborative team environment.
