What you'll be doing
To provide safe, compassionate and high-quality person centered assessment and case coordination to clients.
Ensuring care is delivered in collaboration with clients, their families/carers and a multidisciplinary team to achieve identified goals and outcomes in line with MNCLHD Aged Care Assessment Service guidelines (ACAS).
About you
Do you have demonstrated ability to conduct comprehensive clinical aged care assessments and problem solve complex cases with a focus on achieving positive client and carer/family outcomes while ensuring a person centred approach within a multidisciplinary team environment and demonstrated knowledge, skills and ability working within an acute and/or community setting including an understanding of the principles of reablement, rehabilitation, Geriatric conditions and chronic disease.
