**Community Worker (Full-Time)**

**The Role**

We are seeking a passionate and dedicated Community Worker to join our team in Queanbeyan, NSW. In this role, you will work directly with participants to provide support, advocacy, and assistance in accessing community services, helping them achieve their goals and enhance their quality of life.

**Responsibilities**:

- Supporting participants in developing life skills and independence through structured programs and activities
- Monitoring participants’ progress and outcomes as well as maintaining accurate records of participant interactions
- Working collaboratively with families, carers, and other service providers to ensure coordinated support for participants, including referrals to appropriate community services for additional assistance if needed
- Identifying and responding to participants' changing needs through interviews and other communicative means to assess the nature of their circumstances and adjust their support plans accordingly
- Contributing to the development and implementation of community engagement strategies
- Participating in case conferences and team meetings to ensure quality service delivery
- Support participants in crisis situations by implementing effective care and intervention strategies.

**Skills and Experience**
- At minimum, a Certificate IV in Community Services, Disability, or related field
- Minimum 2 years' experience working in community services or disability support
- Strong understanding of the NDIS and community service sector
- Demonstrated experience in case management and person-centered practice
- Excellent communication and interpersonal skills
- Strong problem-solving abilities and initiative
- Cultural awareness and sensitivity
- Current driver's license and access to a reliable vehicle
