**Meaningful Life - Dementia Memory Care Neighbourhood Coordinator**
**Ballarat**
**Permanent full time + ADO position available**$32.62 per hour**

**About the role**
The Meaningful Life Dementia Memory Care Neighbourhoods Coordinator is responsible for the development, implementation and coordination of meaningful engagement, that gives purpose to our residents living with dementia. They will influence and enrich the quality of life for residents, bringing joy to all they care for. This will be delivered through meaningful engagement that creates purpose, supporting a holistic wellbeing.

**Key Responsibilities**
- Develop, implement and coordinate meaningful engagement the gives purpose to our residents living with dementia, across our Dementia Care Neighbourhoods, that include activities that reflect a person-centred model of care, in line with the Aged Care Quality Standards.
- Participate in the coordination of new and/or existing meaningful activities and person-centred engagement, that reflect the resident’s history which meet residents’ goals, needs and preferences.
- Access residents with responsive behaviours related to dementia and provide resources and support.
- Serve as an expert resource on dementia, offering support, training, and mentorship to team members, promoting a culture of continuous learning and growth.
- Support the residents, family and team with the transition of new residents into our memory care neighbourhoods.
- Coordinate and be responsible for all meaningful life, assessments, care plans, resident iCare profiles and documentation.
- Provide leadership, mentoring and support to the meaningful life team, volunteers and students in the memory care neighbourhoods.
- Coordinate rostering, Optima, training and professional development for direct reports.

**Skills and experience**
- Hold a Certificate III in Aged Care (or equivalent) or Certificate IV in Leisure and Health (or equivalent).
- A Qualification in dementia care is desirable.
- Demonstrate an understanding of Aged Care Quality Standards.
- Understanding and knowledge of people living with dementia.
- Excellent communication and organisational skills, with strong computer and customer service skills.
- Experience with the Aged Care software program iCare is desirable.
- Victorian Driver’s Licence.
