We are seeking a suitably qualified **Leisure and Lifestyle Coordinator** for our Moonee Ponds Home.

**About your role**:
As a **Leisure and Lifestyle Coordinator**, you will support and deliver the Lifestyle program whilst understanding and considering, the evolving needs of our residents. You will work closely with your immediate team, Kitchen/Catering and regular guidance from the Home Manager.

**Key responsibilities**:

- Undertake comprehensive lifestyle review of each resident
- Write ‘lifestyle’ care plans for individual residents
- Lead a team of Lifestyle Assistants
- Utilise community support programs
- Provide end of month reporting /analysis to support the Lifestyle Program
- Collate Surveys and Audits in support of the Lifestyle Program
- Provide ongoing education and support within your team

**About you**:

- Excellent interpersonal and communication skills
- Positive and proactive work ethic
- Relevant qualifications - Certificate IV in Leisure & Lifestyle (or working towards)
- Previous experience within an Aged Care setting
- A genuine interest in elderly care and the desire to improve the lives of our residents
- Intermediate systems/computer skills

**Essential Requirements**:

- NDIS Workers Screening Check Clearance (or willing to obtain)
- Current Police check clearance (or willing to obtain)
- Proof of Work rights in Australia
- Proof of COVID Vaccination certificate (2 doses + booster dose)
- Proof of Flu vaccination (current year)
