What your day will look like....
In this dynamic role, you will work closely with healthcare teams to identify and address barriers affecting patient discharges. Providing expert clinical consultation, you'll develop tailored strategies for patients with complex needs while advocating for their involvement in care decisions. Each day, you'll analyse data to improve services, engage in research, and mentor colleagues, all aimed at enhancing patient flow and experience within our facilities.
