My Choice Support Care is seeking applicants to provide care to our clients - Melbourne North. Our clients require support with personal care and general daily living tasks, and we would love to have a support team that commit to the program to assist with continuity of care.

Our team independently deliver the best possible standards of care to clients in their own homes. You will have an opportunity to truly enhance the day-to-day lives of our clients and make a positive impact. Your work will be highly rewarding with each day varied and different. You will be paid from the start to end of each shift including your travel time and km's between clients.

**For this role you need**:

- Cert III or Cert IV in Disability or relevant qualification* and/or equivalent experience.
- Current police check (less than 6 months old)
- Working with Children Check.
- Current First Aid and CPR
- Current Australian driver’s license with full insurance
- Reliable vehicle

**Typical duties will include**:

- Personal care
- Daily Living activities and routines
- Meal preparation & occasional cooking
- Light domestic assistance
- Participate in social and community activities
- Medication monitoring

Keep accurate records and complete progress notes

Work within the 7 Elements of the NDIS Code of Conduct

**Job Type**: Casual

**Salary**: From $42.00 per hour

Disability Support Worker: 1 years or above

Contract length: 36 months
Part-time hours: 30 per week
