**Your part in The Wesley Hospital team**
- Partnering with our discharge planners, you will manage a diverse case load ranging from future planning and financial advice, life transition for chronic disease, grief and loss adjustment to illness and disability, crisis and conflict management as well as interventions for a range of individual and social challenges.-
- Your interpersonal skills allow you to build strong relationships and credibility with patients, enabling greater effectiveness in your case management and a history of successful patient outcomes. You have a proven ability to deliver a high level of clinical reasoning and decision making that supports your commitment to patient advocacy.**What makes a difference for us?**
- You will have a well-rounded knowledge of best practice approaches to patient care, including up to date knowledge of the government’s My Aged Care and NDIS initiatives as well as:
- Bachelor of Social Work or equivalent
- Membership with AASW
- Minimum of 3 years previous experience in acute Social Work within a hospital environment
- Ability to manage complex and variable caseload
- Commitment to ongoing professional development
- Involvement in quality and service improvement initiatives
- Must hold or be eligible to hold a current Blue Card
