**About the facility**

Post-acute services at Barwon Health provide care to people who have been discharged from emergency, sub-acute and acute services and have been assessed as requiring short term, community based supports to assist them to recuperate in the community and avoid unnecessary representation to hospital.

**About the role**

The Post Acute Clinician will contribute to the safe and timely discharge for people from public emergency departments, acute and sub-acute services, who have been assessed as requiring short term, community-based supports to assist them to recuperate in the community and avoid unnecessary re-presentation to hospital as per Health Independence Program guidelines. The clinician also provides some support to the Information and Access service and other programs to direct referrals to the right care, at the right service and the right time.
