In this role, you'll be managing the implementation and delivery of low level support & maintenance for clients receiving the Bolton Clarke Home Care Packages funded by the Commonwealth Government, Department of Social Services.

**About the role**:
- Providing care coordination, assessment, care planning and review, for low level Home Care Packages (HCP) to ensure effective co-ordination and delivery of care services to clients
- Coordinating the planning and implementation of the HCP within the Consumer Directed Care (CDC) framework which provides choice and flexibility in the way that care and support is provided at home and in an environment of innovation
- Managing a prescribed caseload of low level HCPs including care coordination, assessment and care planning to remain within budgetary constraints of the HCP within the CDC framework program
- Ensuring all reporting and documentation requirements are met, including completing and maintaining timely, accurate and efficient administrative and communication systems
- Maintaining accurate electronic records of client attributable time (both direct and indirect) in the Client Management System
**Qualifications and Experience**
- AHPRA - EN Diploma, or Certificate IV Community Services or equivalent with experience in a similar role
- Demonstrated understanding and knowledge of clients' needs aligned to the various funding programs (e.g. Home Care Packages, Commonwealth Home Support Program, Department of Veterans Affairs, Veterans Home Care)
- Highly developed assessment skills and the ability to creatively problem solve care situations
- Ability to manage a budget effectively
- Case management and community care experience
- Excellent communication and interpersonal skills
- $58,100 - $62,600 + 10.5% Super + $18,550 salary packaging
- Full Time Permanent role covering the Western region
- $500 Commencement bonus + $500 at 3 months
