Care Connect Melbourne is seeking a compassionate and experienced support worker to join our dedicated team. If you’re passionate about empowering individuals to live independently and with dignity, we’d love to hear from you.

**About the Role**: As a support worker with Care Connect Melbourne, you’ll provide high-quality, person-centered care to clients across disability, aged care, and mental health sectors. You’ll work in homes and communities, helping clients achieve their goals and maintain their wellbeing.

**Key Responsibilities**:

- Assist with personal care, mobility, and medication management
- Support clients in community engagement and recreational activities
- Promote independence, safety, and emotional wellbeing
- Maintain accurate records and communicate effectively with the care team

**What You’ll Bring**:

- Minimum 2 years’ experience in a support worker role
- Certificate III or IV in Individual Support, Disability, or equivalent
- Current First Aid & CPR certification
- Valid driver’s license and reliable vehicle
- NDIS Worker Screening Check and Working With Children Check
- Strong communication skills, empathy, and reliability
