**Chronic Disease Coordinator**

**ABOUT THE ROLE**:
An exciting and rare opportunity exists for an experienced Chronic Disease Coordinator to join our Country Health Navigator team. In this work from home role your location is flexible, however the areas of Wheatbelt, Great Southern, Southwest or Perth metropolitan area is preferred.

**This role will also see you**
- working with clients to develop individualised self-management plans and support the implementation of these plans.
- Work across interagency and multidisciplinary teams to facilitate an integrated chronic conditions support
- Develop care pathways in consultation with the client and their medical practitioner when relevant
- Provide referral and referral uptake support in accordance with best practice
- Facilitate effective communication and coordination between all parts of the health systems involved in a client’s care
- Assist clients to access appropriate support and coordinated health care in the community
- Develop new ways of working in a virtual environment

**To be successful in this role, you will also have**:

- Relevant demonstrated experience in Nursing or Allied Health Profession
- Certificate of completion of the Flinders Chronic Conditions Management Program is an advantage but not essential
- Current Working with Childrens Check
- Demonstrated understanding of and experience in chronic conditions and disease management.
- Well-developed communication and interpersonal skills to liaise with clients, carers, families and other health care professionals.
- Demonstrated ability to provide education and support to clients and their carers.
- Intermediate - Advanced knowledge of Microsoft Suit.
- Effective Telstra Internet connection for working at home
