**JOB Title: Disability Support Worker**

**JOB TYPE: Part-time**
**LOCATION: Office and Clients site**

**MAIN DUTIES/RESPONSIBILITIES**
- Support clients to participate in age-appropriate activities and tasks including employment, in-home support, and community access.
- Support clients to access activities and outings in the community.
- Provide direct assistance to each client in accordance with their service plan and as directed
- Work in accordance with the medication administration policy, work health and safety procedures, and client rights policies including privacy and confidentiality.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in client notes and communication book as required.
- Participate in the Lifestyle Service Planning process for individual clients and monitor outcomes.
- Work with clients, family/ carers, relevant case managers, and other professionals to support the client's individual goals.
- In conjunction with other staff members, assist in developing, implementing, and evaluating training and behavior programs to attain identified individual goals.
- Ensure that all programming undertaken is appropriately maintained, documented, and reviewed.
- Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
- Liaise extensively with other staff to ensure the service's overall smooth and consistent running.
- Report regularly to the manager
- Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes, and policy review.
- Provide backup and support to other staff, as necessary.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Notify the service if you are unable to work your rostered shifts, with as much notice as possible.
- Contribute to Case reviews with Employment Consultants and Facilitators.
- Take reasonable care for his or her own health and safety
- Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons.
- Report any quality and safety incident or hazards at work to their Manager or Supervisor.
- Carry out their roles and responsibilities as detailed in the relevant policies and processes.

**QUALIFICATIONS**
- A minimum of a certificate 3 in Disability, Individual support, or Aged care
- Current Valid Driver's license and a registered vehicle( including insurance)
- Right to work in Australia
- Covid 19 Vaccination
- Current National Police Check/ NDIS Screening
- Working with Children Check.
- First Aid certificate and CPR
- NDIS Workers Orientation
- 100 Points of ID

**Job Types**: Part-time, Fixed term, Casual
Part-time hours: 8-12 per week

**Salary**: $27.00 - $35.00 per hour

Schedule:

- Flexible hours
- Monday to Friday

**Education**:

- Certificate I - IV (preferred)

Licence/Certification:

- First Aid Certification (preferred)
- Working with Children Check (preferred)
- National Police Certificate (preferred)
- Driver Licence (preferred)

Work Authorisation:

- Australia (required)
