**About the program**
The** HOPE (Hospital Outreach suicide Postvention Engagement) Wellbeing Support Service** is a **suicide after-care** service that has been developed to care for people discharged from hospital after a suicide attempt or **crisis**. The HOPE Wellbeing support service endeavors to prevent deaths by suicide by providing **care coordination** and supporting participants to link with supports that help keep them safe and support **recovery**.
**About your new role**
As a valued Support Coordinator, you will be helping participants develop relationships and build support networks to ensure they are fully supported and empowered on their journey to recovery. You will participate in the intake and assessment processes, and support participants and families of participants who have been referred to Wellways from Latrobe Regional Hospital and non-clinical community service.
You will utilise your skills to assist HOPE Wellbeing support participants to develop their safety and support plans, while also providing care for them in the first three months following a suicide attempt to help participants stay safe and connect with essential services.
In this role, you may be required to work across the Gippsland region therefore it is a requirement that you are flexible to travel.
**To be successful in this role**
- Relevant qualifications or commensurate lived experience in working in a support role for people with mental health issues, generally a Diploma or above
- Opportunity for lived experience to be used in this role
- Current valid Driver's License and the ability to undertake travel for the role
- Current Working with Children Check and NDIS Workers Screening Check
- Right to Work within Australia
- Satisfactory National Police Records Check
- NDIS Worker Orientation Module Certificate
