**About the role**

We have a full-time position for a Home Care Package (HCP) Care Coordinator to join our growing Home Care Package team. As a HCP Care Coordinator, you will provide our clients with a high-quality case management service, while collaborating with our National Home Care Package Team.

**Duties**
- Liaise with referring agencies such as ACAT, local health providers and community services.
- Assist with the provision of case management services to clients to ensure all care needs are met.
- Ensure that the delivery of services meets all relevant legislation requirements, including the Aged Care Quality Standards.
- Monitor and maintain budgets, ensuring Care Plans do not exceed budget allocations.
- Identify and implement strategies to promote home care packages to potential clients.
- Consult with the National Home Care Package Team, People & Culture Team and Marketing team to seek advice, and assistance on promotion strategies.
- Work with the Home Care Package Team Manager and extended team to ensure continuous improvement and compliance with Aged Care Standards and Reform Agenda
- Mentor and guide Support Workers to ensure clients receive quality and timely supports
- Liaise with allied health providers and health practitioners or services.
- Develop, review and/or implement training resources for the provision of care services.
- Facilitate and conduct training personal care and clinical staff.
- Provide day-to-day support, advice, and assistance as required.
- Identify additional resourcing needs and provide recommendations.

**Skills & experience**
- Minimum Diploma in Aged Care, Community Care, Nursing and/or Case Management
- 2-3 years experience in Home Care Packages and/or community services
- Yellow and Blue Card
- Current First Aid & CPR
- National Police Check
- Drivers Licence Required
- Comprehensive Car Insurance
- Previous experience providing care coordination.

**Salary**: From $37.00 per hour
