**Your new role**
The primary focus of this role is to provide high quality Support Coordination for NDIS participants and facilitate the implementation of all supports identified in a participant’s plan.
The role is focussed on strengthening a participant’s ability to navigate the NDIS, empowering participants to access NDIS and coordinating supports to enable each person to participate more fully in their community of choice. The Support Coordinator will work collaboratively within the portfolio and with other relevant agencies to provide holistic, strengths based support, information and referral services to clients, through a Care Team Model.The Support Coordinator plays an integral role in the development and achievement of a participant’s goals to maximise opportunity and independence.

You will also be required to:

- Manage a caseload that comprises people with a disability, purchasers, their families and networks who present with a variety of needs or service requests, in accordance with relevant legislation and specified within professional practice guidelines.
- Identify and implement innovative service delivery strategies that empower participants to exercise choice and control to access a range of formal and informal mainstream and community-based supports which will assist them to achieve their goals.
- Develop and use relevant planning tools and practices to enhance service provision, including to develop and record progress of goals for clients that are specific, measurable, achievable, relevant and time limited.
- Continuously seek opportunities to build capacity in participants and their families to navigate the service system and maintain support relationships with the end goal being a reduction in the participant’s need for support coordination, and an increase in personal resilience, social inclusion and the ability to engage independently.
- Maintain current knowledge and provide expert advice to the team, participants, their families and carers on any changes in NDIS regulations.
- Implement, monitor, and review the effectiveness of a participant’s supports and negotiate supports as agreed/funded and/or as appropriate.

**What you'll need to succeed**
- Minimum of Diploma in Community Services, Social Work, Psychology, Occupational Therapy or similar OR Minimum of a Certificate IV in Disability Services or similar; AND
- A minimum of 1-2 years’ experience in case management with the human services or disability sector in Australia.
- NDIS Workers’ Screening Clearance and Working with Children Check (for employment purposes).
- Drivers License.
