**About the role**
As a Clinical Care Facilitator/Registered Nurse with BaptistCare at home you will be responsible for coordinating the delivery of high quality care and services that support our customers to remain living at home. You’ll develop genuine connections with our customers and their loved ones, deliver clinical care, and assist them to implement innovative strategies that will optimise their health and wellbeing and support their independence.

A typical day can include:

- Providing support to clients on the Home Care Packages Program
- Providing guidance and clinical support to Care Facilitators caring for aged care clients
- Working alongside our customers to set individual goals that are realistic and achievable
- Implementation of the latest best practice and evidence-based approaches
- The position is a home office based with support from local site office
- Supporting customers predominantly across South East Sydney but also in the Inner West

**About you**

To be considered, you will need
- Registered Nursing qualifications with current AHPRA registration
- 5 years clinical care experience in an acute and aged care sector
- Proven experience in working with budgets & service delivery parameters
- Demonstrated ability to lead & work as part of a team
- Current NSW/ACT Drivers Licence
- Available for on call as required
- Sound written & verbal communication, and computer skills
