Your role
- Work collaboratively with stakeholders and advocate for the needs of students appointed to their case load
- provision of student profile, and summary to class teachers upon commencement of school year and support any transitions
- Develop, produce and manage student plans and associated resources for those appointed to their caseload - Individual Support Plans, Individual Curriculum Plans (ICP), Personalised Learning Plans, and Master provisions for alternative activities, Transition Plans, and Behaviour Management Plans (IBSP), Social Stories. - Co-ordinate, collect data and write verifications (initial and reviews) See Ed studio for support
- Co-ordinate and collect validation evidence for students appointed to their caseload, which supports the EAP Profile. Review EAP profiles yearly
- Manage case files and information for students appointed to their case load
- Provide support to classroom teaching teams in planning and adjust/modify class content and assessments
- Provide support to class teachers to adjust/modify unit plans and record data
- Collect data and make observations in regards to students appointed to their case load
- delivery and co-reporting on all students in the class. Liaise with Advisory Visiting Teachers, Physiotherapists, Occupational therapists, Speech Language therapists-complete referrals and implement programs. Contribute to excursion and camp preparation - additional support personnel, risk assessments
- Attendance at Year level meetings, planning meetings and participation in moderation
- Create ICP's and attend reviews and initial submission of ICP
