**The Southern Adelaide Local Health Network is seeking to engage skilled and motivated Social Workers to join the Inpatient Rehabilitation team.**

This role contributes to the provision of a Social Work service to inpatients of the Rehabilitation, Aged, and Palliative Care Services by undertaking casework including psychosocial assessment, counselling, advocacy, discharge planning, networking and education.

Currently, this is a term vacancy until December 2024 with the possibility of extension.

Your role **responsibilities** may include:

- Conducting thorough psychosocial assessments and providing interventions that work to enhance the quality of life and wellbeing of patients receiving inpatient rehabilitation after a significant medical event;
- Work as part of a multi-disciplinary team
- Responding to referrals and contributing to the formulation of plans to assist clients and their carers in rehabilitation, to discharge from hospital and to support their safe and timely reintegration back into the community
- Advocating for and on behalf of individuals and groups including ensuring the exchange of relevant information about client and carer needs as well as optimising clients’ and carers’ use and access to health, government and community resources

- Ability to communicate with a wide range of client and professional groups.
- Self motivation, initiative and perseverance.

A degree or equivalent qualification which entitles registration as a Social Worker with the Australian Association of Social Workers is essential for this role. For full details, please see the attached Role Description.
