The successful applicant must have a diverse understanding of cultural and local knowledge.

Your responsibilities will be to navigate with participants the National Disability Insurance Scheme for the benefit of participants needs in line with their goals and implement supports relating to their unique circumstances.

You will have an understanding of rules and regulations in accordance with all relevant legislation.

You will have a clear understanding of participants routines, preferences and goals.

You will have the ability to manage and track participants wellbeing, and appointments, ensuring timely followups.

You will be highly dedicated to MLH Care, participants and the NDIS structure, policies and guidelines to develop and implement NDIS plans to meet unique goals of the participant/s.

You will be able to self manage your time efficiently and promote the purpose and direction of MLH Care to best support all participants and stakeholders with in the greater Shepparton region.

You will have, and be able to build on a strong relationship with local agencies, including but not limited to, the NDIS LAC, families, allied health professionals, and other stakeholders in and around Greater Shepparton.

You will have the ability to manage and complete reports for both MLH Care and NDIS participants with Support Coordination or Psychosocial Recovery Coaching.

You will have experience managing Key Performance Indicators for best practice.

Must have:
Valid Drivers License.

Working With Children Check.

Police check less than 3 months old.

NDIS workers screening check.

Qualifications in Support Coordination and Psychosocial Recovery Coaching.

A mínimal professional on the job experience of no less than 1 year in either Support Coordination and Psychosocial Recovery Coaching, Social Work, Or Psychology.
