This position provides culturally sensitive support to Aboriginal and/or Torres Strait Islander women during pregnancy, as well as to women whose babies identify as Aboriginal and/or Torres Strait Islander. It involves assisting families throughout the pregnancy journey by facilitating informal support groups and helping them access appropriate services. Support is delivered through various methods including telephone, telehealth, home visits, and in hospital or community settings to ensure accessible and responsive care.
The role also focuses on improving health outcomes and wellbeing for Aboriginal and/or Torres Strait Islander families on the Central Coast with children aged 0-5 years. The Aboriginal Health Worker collaborates with families, Registered Midwives during the antenatal and postnatal periods, and Child and Family Health Nurses until the child reaches school age. This partnership is guided by the Ngiyang Aboriginal Pregnancy, Child & Family Health Service Model of Care, ensuring culturally respectful and holistic care navigation throughout early childhood.
**About the Role**
