**Position Details**

**Key Responsibilities**
- Plan, implement, and deliver structured group activities that promote social participation, wellbeing, and reablement.
- Tailor activities to meet the diverse needs, interests, and abilities of older clients, including those living with dementia.
- Provide specialised support during group sessions, ensuring each client’s dignity, safety, and goals are upheld.
- Assist clients to maintain social interactions and relationships, in line with their identified needs, goals and preferences.
- Facilitate client transport to and from group activities where required, ensuring duty of care and safety standards are met.
- Assist clients to collaborate with rest of group to develop plan for weekly community-based social outings and activities, facilitating discussion and planning to develop weekly program.
- Assist group clients to navigate disagreement or conflict, de-escalating and ensuring reasonable and respectful communication among group members to resolve issues.
- Assist group members to understand incidental costs associated with planned or requested activities, e.g. entry fees, food and drink costs, etc.
- Assist clients to develop and maintain positive healthy relationships with service providers, supports, family and friends.
- Support clients in carrying out self-care tasks while participating in group support, and provide guidance or assistance where needed, as per support plan.

- Participate in supervision, and ongoing learning and development activities.
- Promote and maintain a safe and healthy workplace, ensuring adherence to WHS policies and procedures
- Work within the vision, mission, and values of Footprints

**Knowledge & Skills**
- A strong commitment to working with people with a diverse range of needs, which reflects a caring, compassionate, and strengths-based approach to service provision
- Experience supporting people with varying needs including psychosocial, mental health, housing insecurity and financial disadvantage
- Self-motivated with the ability to work with mínimal supervision and use initiative; able to prompt and guide discussions around program planning.
- Positive and effective interpersonal skills, with the ability to facilitate group discussion and de-escalate and manage emerging issues.
- Strong organisational skills and attention to detail in documenting client information
- Effective written and record-keeping communication skills
- Minimum Certificate III in Individual Support, or other relevant field
- Current class C driver licence with your own reliable registered and comprehensively insured vehicle
- National NDIS Worker Screening Check or current Yellow Card

**Desirable**
- Current First Aid and CPR certificate
- Certificate III or IV in Individual Support, Community Services, Leisure and Health, or similar qualification.
