**Overview**

**Key Responsibilities**:

- Assist with the provision of high-quality care that is tailored to the individual needs of clients, considering their care plans, available funding, and services.
- Help manage various administrative tasks, such as inputting data, managing phone calls, and scheduling appointments with Allied Health professionals.
- Collaborate with community service providers to establish a network that offers comprehensive care options to clients.
- Maintain accurate and confidential client records, keeping them up to date and following professional standards.
- Work independently and as part of a small team, taking ownership of assigned tasks and actively participating in team meetings and planning sessions.

**Qualifications and Skills**:

- Own registered vehicle and current driver's license
- Strong organisational and time management skills, with the ability to multitask and prioritise.
- Excellent communication skills, both written and verbal, with the ability to effectively engage with clients and service providers.
- Proficient in using Microsoft Words, Excel and PowerPoint.
