The in-home aged care division of the organisation provides services to aged people across Sydney delivering Home Care Packages and CHSP with a team of passionate people who strive to support aged persons to stay at home.

**Role**

As Care Coordinator, you will be managing your case load of low level of needs clients on the Commonwealth Home & Support Program (CHSP) as well as some low needs Home Care Packages (HCP) Level 1 clients.
You will thrive in a fast paced and multi-tasking role as you will be ensuring your clients are receiving the highest level of quality care & needs.
Your role will include:

- Contacting clients predominantly by phone to ensure the highest level of care, respect and quality are delivered - you will visit your clients every now and again
- Organising Assessments, Care Planning & Reviews
- Making sure all client notes are accurately recorded in client database
- Liaising with stakeholders such as GP's & allied health professionals

This role is a 12 month contract.

**Team**

As Care Coordinator you will be joining a vibrant & passionate team of 50 Care staff across Sydney, with your direct team consisting of 8 in your allocated area (South Sydney/ Wollongong).
You will be reporting to a very supportive Manager who will nurture and support your career growth with monthly formal catch-ups plus a very available policy for ongoing communication.

**About You**
- You would have had experience in in-home aged care care management as either a Coordinator, Scheduler or Support Worker and are looking to grow your career
- You have a knowledge of Home Care Packages / CHSP
- You are able to travel now and again to client meetings with a valid drivers licence
- You thrive in a fast paced environment
- You want to work in a friendly and passionate team of people
- You live in reasonable driving distance to the Wollongong hub office
