An opportunity to join Western Health Mental Health & Wellbeing Service's Community Program based in Sunshine.

A full-time, ongoing position is available in our Community Mental Health Team. The Community Team is an integrated program with sub-acute community response over a 7 days week. Monday to Friday functions include intake/triage, case management and therapies. This is a multidisciplinary role.

**A typical day**:

- Assessing and providing interventions to consumers with various psychiatric illnesses
- Coordinating medical, psychosocial and legal aspects of care delivery
- Engaging with consumers, their family members and other service providers
- Development and delivery of evidence-based practice and appropriate treatment options
- Working in collaboration with your multi-disciplinary mental health team

**To be Successful in this Role, you will**:

- Lead with kindness, role modelling open communication and a desire to support and enhance clinical care across the team
- Contribute effectively to a multidisciplinary team with a genuine commitment to working within a client-centred framework
- Have knowledge and practice in delivering mental health care
- Have consolidated skills in assessing mental state and risk
- Be collaborative, responsive and empathic
- Have the capacity to consult, liaise and negotiate sensitively with clients, their family or carers, treatment teams and community services
