The Salvation Army Australia’s Homelessness Stream provides high quality, person-centred services to people at risk, and or experiencing homelessness. We support all cohorts in case management, outreach support, financial assistance, connection, and referral to other specialist services.

We are currently seeking a value driven and proactive Case Manager to provide, as part of a team, case management support, in a community setting, that assists people to achieve housing stability, experience improved social inclusion and achieve a level of self-mastery that will enable them to sustain tenancy and lead their best lives. This support is achieved through person-centred case management incorporating resilience, trauma informed and strengths-based practices whilst working effectively with other members of the team to provide a comprehensive service that reflects the vision, mission, and values of The Salvation Army.

The role also provides information, advocacy and linkages into community supports and programs. Work collaborates within the broader programs of The Salvation Army (TSA) and external Specialist Homelessness providers within the Homelessness wider sector to provide an integrated support response to the target group.

This position is a 6-month contract, full-time position and is based in Toowoomba.

You will successfully
- Establish effective relationships with clients that encourage self-determination and participation
- Provide case management support to individuals
- Complete thorough assessments, inclusive of needs, risks and vulnerabilities
- Ongoing assessment of the individuals’ presenting issues, vulnerabilities and needs from initial intake to exit
- Develop individual case plans in collaboration with clients and include a range of interventions that address the needs or goals of clients, as well as barriers to achieving those
- Assist clients to access support services and establish community supports, and where needed are assisted through advocacy with service providers
- Progress and monitor outcomes of individual case plans and where needed action is taken to ensure the plan continues to meet client needs or goals
- Service gaps are identified and recommendations for service delivery improvements are made

You will have
- Tertiary qualification in Community Services, Social Work or related field. A degree level qualification is preferred.
- Relevant experience in a social service environment including in client assessment and case management
- A valid Employee Working with Children Check (Blue Card)
- A First Aid Certificate and CPR
- A QLD Driver's licence
- A willingness to undergo National Police Check
