**About the role**

We have a permanent full-time role available for a Client Liaison Officer who is a go-getter and has solid experience in budgeting, case management and care planning.

**Your day to day role** will include but not limited to:

- undertake assessments, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client in achieving their identified goals.
- Supporting the client to manage and monitor their budget to ensure care packages are financially sustainable and
- monitor the services provided to ensure these are following relevant service models and guidelines.

**bout you**
You will have a background in Community Services, Aged Care or Disabilities.
- Certificate IV in Health, Community Services, Aged Care or Disability Support
- Proven understanding of assessments, packaged care and case management
- Ability to engage clients and work effectively with them to achieve identified goals
- Support and guidance to clients with budget management and financial sustainability
- Exceptional communication skills
- Ability to advocate strongly for your clients
- A sound understanding of the community care system
- A current National Police Clearance
- Current COVID Vaccinations
- Current Qld Drivers Licence
