**Your Opportunity**:
The position of Senior Social Worker (Adolescent Integrated Care) will:

- Deliver high level, evidence based, tertiary clinical social work interventions for an adolescent (12-18 years) cohort, where there is identified social complexity and assessed risk of social admission, and/or re-presentation to the Queensland Children's Hospital (QCH) Emergency Department (ED) due to social factors.
- Work closely with the Emergency Department team, supporting teams to promote integrated systems responses, trauma informed education and evolving model of care.
