**_About the role_**
The ACAT assessor stands within the nursing department of the CCH/ACAT multidisciplinary team. The primary purpose of the position is to provide holistic, comprehensive assessment to Clients referred for the Transitional Aged Care Program and to the target population in the St George Geographical area in the community, hospitals and aged care facilities as applicable.
Key Responsibilities
- Maintain ACAT outputs as per requirements
- Provide holistic and comprehensive assessment strategies to meet the need of the target population
- Comply with documentation standards and guidelines for entries into the electronic medical record
- Maintain CPD points with relevant education
- Participate in a Quality Activity that is relevant to the clinical area
**_What you bring_**
Criteria 2: Minimum 3 years’ post graduate experience working with older people in an acute, aged care and/or community setting.
Criteria 3: Knowledge of Aged Care Systems as well as Aged Care issues and experience working with clients from diverse cultural backgrounds
Criteria 4: Demonstrated ability to provide education to clients, peers and carers as well as commitment to ongoing self
- education and best practice.
Criteria 5: Demonstrated high level of interpersonal, written and verbal communication skills including computer literacy.
Criteria 6: Demonstrated ability to work autonomously and as part of a multidisciplinary team.
Criteria 7: Demonstrated knowledge of Quality Improvement initiatives and Work Health and Safety principles relative to community service delivery.
