Ungooroo Aboriginal Corporation is seeking an experienced Outreach Worker as part of the
Integrated Team Care program. The Outreach Worker will provide non-clinical services that help
local Aboriginal and Torres Strait Islander people make better use of available health care services.
This is an Identified Position for an Aboriginal or Torres Strait Islander person.
The role is being offered on a fulltime basis.
Responsibilities include
- Community Liaison to establish links with local

Aboriginal and Torres Strait Islander communities
to promote the importance of improving health
outcomes and encourage and support the
increased use of health services.
- Provide practical assistance to access services and

attend appointments including GP care planning
and assessments, follow-up care, transport.
- Distribute information to Aboriginal and Torres

Strait Islander people about how to access available
services (eg care coordination, relevant local health
services, registration for a Medicare card)
- Provide support for outreach/visiting health

professionals where required
- Identify and provide feedback for the HNECC PHN

regarding barriers to health services for Aboriginal
and Torres Strait Islander people
- Work in collaboration with the Care Coordinator
- Maintain privacy and confidentiality around patient

personal information
- Data Collection & Reporting
- Understanding of WHS requirements and

compliance

provision with genuine empathy and interest in patient
needs.
- Excellent interpersonal and communication skills across

all ages and social groups.
- Maintain absolute confidentiality around patient and

practice information
- Demonstrate a knowledge of, and compliance with, all

relevant legislation and common law obligations
affecting nursing and patient care.
- Maintain a vigilant attitude to accuracy
- Ability to work cooperatively, independently, and to

prioritise and organise.
- Be always well-presented, friendly, courteous, and

obliging.
- Always represent the practice in a confident and

positive manner.
Selection Criteria
- Certificate III Aboriginal Health Worker or working

towards achieving the relevant Qualification.
- Strong links with the community in which they work

Children Check and a Criminal History Check
This is an Identified position under Section9A of the NSW Anti-Discrimination Act 1977

**NOTE**: Being Aboriginal or Torres Strait Islander is a genuine occupational qualification for this position as described
under Section 14(d) of the Anti-Discrimination Act 1977
