Peninsula Home Hospice (PHH) is an accredited, not for profit community palliative care organisation
providing specialist health care and practical support to people living at home with a life limiting illness and
their families and friends. PHH supports people who live within the Mornington Peninsula Shire, City of
Frankston, and City of Kingston (south of Mordialloc Creek)

**Reports To **Clinical Manager - Nursing

**Award **Nurses Award 2020 (Private)

**Classification **Grade 4A

**Hours **As per contract

**Role Reflection **Initially during the probation period at three months, and six months, then annually.

**_Key Selection Criteria: Qualifications, Knowledge, and Experience: _**

**Mandatory **- Registered Nurse Division 1 with current certification from the Australian Health
Practitioner Regulation Agency (AHPRA)
- Certificate IV in Workplace Training and Assessment (minimum)
- Experience and proficiency in developing and facilitating multi-modal education

and training programs
- Experience working in palliative care sector with a highly developed

understanding of the palliative approach and specialist palliative care
- In-depth understanding of the issues encountered by clinicians who provide

support to people living with a life limiting illness as well as the issues clients and
carers encounter
- Excellent interpersonal and written communication skills.
- Ability to work independently with mínimal supervision and as part of an

interdisciplinary team.
- Well-developed computer skills: familiarity with the Microsoft suite of programs

and electronic medical records.
- Compliance with vaccination requirements.
- Current Victorian Drivers Licence.
- National Criminal Record Check and Working with Children Check (WWC) prior to

commencing employment.

- Post Graduate qualification in palliative care
- Experience working within a community setting
- Experience in the provision of supervision, mentoring and performance

management of staff
- Demonstrated experience in building and maintaining relationships with external

stakeholders

**_Position Objective _**
- The focus of the Clinical Educator - Community Palliative Care (CE-CPC) role is on the

identification, development, facilitation and evaluation of educational programs and resources that
support the learning and professional development needs of the interdisciplinary team to ensure
competence and confidence working within community specialist palliative care.
- Working with the relevant Manager/s and the Human Resources Team, the CE-CPC is integrally

involved in the provision of support, guidance and educational opportunities for clinical, and non
- clinical, team members new to Peninsula Home Hospice.
- Engaging with external stakeholders to identify, develop and facilitate palliative care education that

may be required.

**Key Responsibility 1 Clinical Education**:
**KPI**:

- Collaboratively engages in the identification, development and provision of

education programs that reflect current evidence-based palliative care practice
for the PHH clinical team and external service providers.
- Adopts a novice to expert approach to meet the learning needs of the clinical

team.
- Provide training and education programs utilising multimodal approaches.
- Ensure education programs and resources are evaluated and refined.
- Support new clinical team members through the onboarding and orientation

program.
- Provide support, guidance and mentoring opportunities to clinical team

members to expand and embed their knowledge and skills through partnering
on clinical visits and through clinical discussions.
- Participate in an annual revision, or when deemed necessary, of the education

and training program to ensure alignment with organisational goals, clinical
needs and legislative requirements.

- Provides leadership, education, and mentoring opportunities for PHH clinical

team members.
- Work as part of a team to develop, deliver or facilitate the clinical education

needs of the non-clinical workforce.

goals and values.
