WHY THIS ROLE MATTERS
Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.
We are seeking a values-driven and proactive Case Manager, reporting to the Team Leader Supported Independent. You will provide intensive case management to young people between the ages of 16 and 18 years of age. Supporting young people transitioning into independent living and self-reliance through the provision of intensively supported accommodation, advocacy and support developing essential independent living skills across all life domains.
This is a permanent full-time position located in Pooraka.
THE ROLE YOU COULD PLAY
- Provide support to young people in line with the Youth Service Model of Care
- Build effective relationships with clients, encouraging self-determination and participation
- Provide ongoing assessment of our client's issues, needs and circumstances from initial intake to exit
- Ensure clients have safe accommodation, on-site supervision, guidance and support across all life domains
- Provide clients with constructive advice on budgeting, shopping, cooking/nutrition, household cleaning, building relationships with neighbours and other community members
- Maintain accurate and up to date client records including case notes and data
- Participate in daily handovers when rostered on shift
WHAT YOU WILL BRING
- Degree qualification in social work, welfare, community development or related field, certificate IV in Youth Work or equivalent.
- Relevant experience in a social service environment
- Willing to complete a Psychological Assessment (in line with Children and Young People Safety Act 2017)
- Through their Eyes (Reporting Abuse and Neglect) Training
- First Aid Certificate
- A current Working with Children's Check
- A willingness to undergo a National Police check
